ResponsibilitiesJob SummaryThe Pharmacy Medication Reconciliation Technician will improve medication safety by assisting pharmacists in collection and documentation of up-to-date medication lists for patients being admitted to the organizations hospitals in order to optimize the medication reconciliation process and reduce the number of medication errors secondary to inaccurate or incomplete information.
Responsible for obtainingand verifying medication history in the electronic medical record in an accurate and timely manner in compliance with standard procedures and regulatory standards.
Responsible for accurately documenting medication lists into the electronic health record, communicating to the verifying pharmacist, and relaying any outstanding transition of care issues to the pharmacist and/or provider in a timely manner.
Performs other duties as assigned.
QualificationsEducationHigh School Diploma or GEDRequiredExperience< 10 months pharmacy technician experience or completion of a formalized technician training program.
Required Licenses and CertificationsCertified Pharmacy Technician (CPT) Current license by AZ State Board of Pharmacy.
RequiredPharmacy Technician Certification Board (PTCB) Current Pharmacy Technician Certification (CPhT) issued by Pharmacy Technician Certification Board (PTCB).
Required Recommended Skills Certified Pharmacy Technician Communication Regulatory Compliance Estimated Salary: $20 to $28 per hour based on qualifications.

The check will bounce and the victim is left responsible.
